An interprofessional Primary Care Home designed to provide longitudinal medical management for patients across the Neuro-Continuum.
We serve as a "Stabilization Hub" for complex patients who require specialized insight and interdisciplinary support.
Accepting referrals for permanent "attachment" or "consultative co-management."
